Invest in your well being
Seeking support is an investment in your health, relationships, and future. My goal is to provide thoughtful, individualized care in a setting that allows for meaningful therapeutic work while maintaining flexibility for clients with a variety of payment preferences.
Questions about insurance, private pay, or out-of-network reimbursement are welcome. If you are unsure about your options, I am happy to discuss them and help you determine the approach that best supports your needs and goals for treatment.
-
I am in-network with select insurance plans (Highmark BCBS, UMR, PEIA, The Health Plan, United, and Peak). Coverage and benefits vary by policy, and clients are encouraged to verify their mental health benefits directly with their insurance carrier prior to their first appointment.
If I am not in-network with your insurance plan, you may still be able to use your out-of-network benefits.
-
If I am not in-network with your insurance plan, I can provide a detailed superbill that you may submit to your insurance company for possible reimbursement through your out-of-network benefits.
Reimbursement is determined by your insurance plan and cannot be guaranteed. I encourage clients to contact their insurance carrier to ask about:
Out-of-network mental health benefits
Deductible requirements
Reimbursement rates
Submission procedures

Many clients choose to pay directly for services rather than use insurance. Private pay offers increased privacy, flexibility in treatment planning, and freedom from many of the limitations that can accompany insurance-based care.
Initial Assessment $250.00
Individual Sessions $200.00
Family/Parent Consultation $200.00

Payment is due at the time services are rendered unless other arrangements have been made in advance. Major credit cards, HSA/FSA cards, and other accepted forms of payment may be used.

Under the law, health care providers are required to provide individuals who are not using insurance, or who are not submitting claims to insurance, with an estimate of the expected charges for medical and mental health services.
You have the right to receive a Good Faith Estimate explaining the anticipated cost of your care before services are provided.
The Good Faith Estimate will include expected costs for services provided by Revise Counseling & Consultation LLC and is based on information known at the time the estimate is created. Actual charges may vary depending on the course of treatment and services provided.
